1.Females: DS-156 APPLICATION FORM (https://evisaforms.state.gov/ds156.asp)(completing the form on-line, and must take the printed two application forms with bar code)
2. Males: DS-156 APPLICATION FORM (https://evisaforms.state.gov/ds156.asp)& BS-157 APPLICATION FORM (http://www.state.gov/documents/organization/7769.pdf)must take the printed
3. 其它必带材料 1. U.S. $100 CASH
2. 护照 & 学生签证-------------------------（最好自己做一份Copy留档）
3. 回邮快递信封一个-----------------------------------（小的那种就可以了）
4. 在校证明----------------------（B1 is for Business Trip, B2 is for traveling）
5. 成绩单---------------------（从E-service里打印出来，再去Office盖章）
6. 两张护照照片--------------------------------------（6个月之内的 & 两寸）
7. 资金证明------------------------------------------（Monthly bank statements）

办理这些之前，您的学生签证和护照的时间一定要大于6个月。 如果不是，就没办法过关。所以请先办理您的学生签证和护照！
